Orthodontic treatment moves your teeth into their ideal positions by applying carefully controlled forces over time. When treatment ends, the bone and soft tissue surrounding your teeth are still adapting to those new positions. Without a retainer, teeth will drift — sometimes significantly — especially in the first 12 months after treatment.
This is not a flaw in treatment. It is simply how teeth and bone work. The solution is retention: wearing your retainer consistently, especially in the first year, and keeping your retainers in good condition for life.
The most critical retention period. Wear your retainer every night without exception. This is when teeth are most likely to shift.
Continue nightly wear. The bone around your teeth is still consolidating. Skipping nights occasionally is less risky at this stage, but consistency is still the best policy.

Custom-fabricated clear retainers that fit snugly over your teeth. Virtually invisible when worn. Removed for eating, drinking, brushing, and flossing. Most patients receive clear removable retainers at the end of treatment. These are the retainer type covered under the SPOCS Retainer Program at $50/arch replacement.

A thin wire bonded to the back of your lower (and sometimes upper) front teeth. Completely invisible from the front. Stays in place permanently — no compliance required. Often used for lower front teeth where relapse risk is highest. Requires special flossing technique around the wire. Dr. Mansour recommends bonded retainers when they are clinically appropriate for your case.
The SPOCS Retainer Program costs $750 for five years of coverage. Replacement retainers are $50 per arch under the program, compared to the standard price of $225 per arch. The program fee is FSA and HSA eligible and is processed through OrthoFi.
Clear removable retainers typically last 1 to 3 years with regular use before they show wear, crack, or lose their fit. Bonded fixed retainers can last many years but may need repair if the wire comes loose. Having a replacement plan in place from day one is the smart approach.
